LEAD MDS COORDINATOR
Overview:
MDS nurses gather information on the facility’s current residents for future assessment, including physical and mental states. They assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents. MDS nurses coordinate the shipment of their facility’s services to its residing residents.
MDS Nurse Skills :
- Conduct Patient Assessments
- Develop Patient Care Plans
- Make Patient and Facility Recommendations
- Oversee Medicare and Medicaid Programs
Highlights:
- Hourly position (40 hours; $40-45+ an hour, DOE), with flexible hours at the SNF (not hybrid, nor remote)

- No on-call
License & Certification:
- Active/unencumbered RN/LPN state license
Education, Training & Experience:
- 2-3 years of Lead MDS experience in long term care
- Proven track record of capturing maximum reimbursements
- Knowledge of Matrix and RAC-CT
- Requires a can-do attitude
