LEAD RNAC/LPNAC (MDS)
Overview:
- MDS nurses gather information on the facility’s current residents for future assessment, including physical and mental states. They assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents. MDS nurses coordinate the shipment of their facility’s services to its residing residents.
MDS Nurse Skills :
- Conduct Patient Assessments
- Develop Patient Care Plans
- Make Patient and Facility Recommendations
- Oversee Medicare and Medicaid Programs

- This Lead role has the support of another LPNAC (LPN) MDS Nurse, as well as an amazing Regional RNAC Specialist, and Remote Case MGT Team
- Salaried position (40 hours; $80-90K, DOE), with flexible hours at the SNF (NOT hybrid, nor remote)

License & Certification:
- Valid Pennsylvania RN/LPN license
Education, Training & Experience:
- 2-3 years proven MDS experience capturing maximum reimbursements
- A working knowledge of Matrix and RAC-CT is highly preferred
- This pivotal role also requires a can-do attitude
